If you will be conducting your study at any of the Sentara locations and/or accessing any Sentara medical records you must obtain approval from Sentara. Use this link to complete a Research Request Form and to read about your responsibilities as an investigator involving Sentara facilities and/or data.
To request access to Sentara data:
- Complete the VHS Data Request Intake Form.
- Request data through SENTARA Research Data Request Portal.
- Complete and upload a SENTARA Data Use Attestation.
- For projects involving any trainees, there must be a VHS faculty mentor as the PI.
For projects involving medical students, the project must be cleared through Medical Student Research
