If you have been immunized at a location other than Occupational Health, you must upload documentation to the VAX Portal using the following instructions:
Vax Portal Instructions
- Select Entry Type field and then select appropriate designation.
- Complete additional fields (Note: Employee Number is the number on the front of your ID badge).
- Under Vaccination Details, enter date vaccine administered.
- Under Vaccine Documentation, click Attach Proof of Vaccination.
- Click Upload, attach appropriate file, click Save.
