Notice: MachForms (forms.odu.edu) has been taken permanently offline due to a security issue. Anyone attempting to access or submit a form built at forms.odu.edu will encounter an error message and be unable to complete their submission.
All active forms must be migrated to Kuali
If you have online forms that need to remain active and accessible, they will need to be recreated in Kuali. There are two ways to accomplish this:
Option 1: Request Assistance (Recommended)
Even though the MachForms front-end is down, our team still has backend access to view your original forms and field structures. To have us help recreate your form and get you back up and running as quickly as possible, please fill out our Migration Request Form. Be sure to include as much detail as you can.
Option 2: Build It Yourself
If you prefer to build the form yourself, you can log into odu.kulaibuild.com using your MIDAS ID and password. Review Kuali’s Form Building Overview to learn more about setting up forms in the system.
