REIMBURSEMENT ACCOUNT ENROLLMENT DUE FRIDAY

Friday, Nov. 21, is the last day for 2004 enrollment in Flexible Reimbursement Account(s) for medical expenses or dependent care expenses for an effective date of Jan. 1, 2004.

Don't forget that this is for a short plan year from through June 30, 2004. The minimum contribution will be $10 per pay period not to exceed $2,500 for the short plan year.

If you are currently enrolled in a FRA, you must re-enroll for the new plan year. 2003 FRA accounts will end on Dec. 31.

Enrollment forms are available in the Department of Human Resources on 49th Street, building no. 2.
