HEALTH RECORDS DUE WITH STUDENT HEALTH SERVICES
All first-time, full-time students at Old Dominion are required to submit health history information and immunization records prior to their second semesters or face a hold on their registration.
Effective Feb. 1, holds were placed on the records of students who have not completed and returned the form, according to Anita M. Golemo, a registered nurse clinician at the Student Health Center. They will not be able to register for summer or fall 2001 classes until this requirement has been met, she said.
Students can access the Health History and Immunization Documentation form on the Student Health Services Web site, http://web.odu.edu/studenthealth.
Completed forms can be returned to the Student Health Center in person; by mail to Student Health Services, 1007 South Mall, Webb Center, or fax, 683-5930. The immunization hold will be removed when the required information is completed.
To verify that an immunization hold has been removed, students can check Leo Online or Leo Voice Response at 683-6150.
