COVID-19 Self Reporting

ODU Risk Management has created an online form for self-reporting of travel to areas with higher concentration of COVID-19 cases or for individuals who have been asked to self-isolate or exposed to someone who has been confirmed with COVID -19. Go to the post-travel and/or contact exposure self-report form to complete it, and an Office of Risk Management representative will contact you.
