Sole Source Request Form

Please find attached the revised Sole Source Request form from Procurement Services.  Effective immediately, this form is to be used for all future sole source purchase requests.  Any future submittal for a sole source purchase request using previous versions of the sole source request form will be returned with guidance on accessing the new form. 

Additionally, any current requests that have already been submitted to Procurement Services for review, but fail to substantially justify the sole source request, may be returned with guidance to complete and resubmit the request using the new Sole Source Request form.
