Employee Flu-Shots
<p> If you did not have a chance to receive your <strong>FREE </strong>Flu-Shot last week at the Benefits Fair, you will have another opportunity on <strong>Wednesday, October 17<sup>th</sup> from 8am to 1pm</strong> in the York/Potomac River Rooms in Webb Center.</p> <p> All adult full time employees and family members that are 18 years of age and older and who are enrolled in State Health Benefits will have an opportunity to receive their Flu-Shots free again this year. Participants must present their Member ID card at the time of vaccination. The vaccination will be administered in your arm, please be sure to dress appropriately in a loose fitting long sleeve or short sleeve shirt or blouse.</p> <p> Please print the attached consent form, complete it and bring it with you when you come to receive your flu shot.</p>
