Children normally loss eligibility when:
+ They get married.
+ Join active military service.
+ No longer live at home (unless they are away at school or live with the other biological parent)
+ Receive less than half of their support from you/they are self supporting.
General rule - If you can no longer claim the child as an exemption on your federal taxes, they are probably no longer your dependent and not eligible for coverage under your health plan.

You have 60-days from the loss of eligibility to:
+ You must remove the ineligible child from your health coverage.
+ Change your coverage (add or remove optional features).
+ Increase or reduce you Medical Reimbursement Account
+ Reduce or cancel your Dependent Care Reimbursement Account.
Provide HR with a mailing address for the child. This address is required so that HR can provide the child with COBRA Extended health care information.
