NURS 492: Community Health Nursing

The following guidelines provide information from a typical course syllabus for Nursing 492 and detailed requirements of what you should include in the portfolio. Students who have been approved to complete Nursing 492 via portfolio should contact their faculty assessor to review the portfolio expectations.

Course Description

This course focuses on professional nursing practice with families and communities as clients.  Emphasis is on community wellness, interaction with political influences and epidemiological principles.

Portfolio Guidelines

  • Narrative (Be sure to include these areas. You can also include additional, related areas):
    • Description of your organization, your job title and your role within the organization
    • Description of ways in which you used critical thinking skills along with independent judgement when working with individuals as well as groups
    • Description of ways in which you demonstrated appropriate interventions to maximize the overall health of individuals and groups
    • Description of ways in which you demonstrated communication skills while working with individuals and groups
    • Description of community health teaching projects, showing teaching and learning strategies you have used
    • Description of ways in which you incorporated research-based interventions in the community health field
    • Description of ways in which you functioned as an advocate for community groups
    • Description of ways in which you demonstrated acceptance of responsibility and professional accountability
    • Description of ways in which you demonstrated proficiency in performing nursing interventions adhering to all standards of practice (institutional, legal, ethical, and professional)
  • Supporting documentation
    • Expanded resume with related community health nursing positions, description of activities and responsibilities held
    • Letters from supervisors summarizing your responsibilities in the community health field
    • Evidence of sample teaching projects
    • Samples of projects completed in the job, such as conducting a health fair or screening (comparable to health planning project)
    • Samples of workshops conducted to train staff in community related activities
