COVA Care Optional Benefits
1. Out-Of-Network Benefit
This option allows you to receive covered services at a reduced level from non-network providers—that is, providers that are not in the Anthem, BlueCard PPO, or ValueOptions Behavioral Health networks. The plan payment for covered services is reduced by 25%. You are responsible for any applicable deductible, copayment or coinsurance. You also pay any balance above the allowable charge. Claims payments are made directly to the member when services are received from a non-network provider.
2. Expanded Dental Benefit
Expanded Dental
This option allows you to add Complex Restorative and Orthodontic benefits to your basic dental benefits. The maximum the plan pays each calendar year for the basic dental and complex restorative is $300 more than under the COVA Care basic plan.
The COVA Care basic plan includes coverage for routine diagnostic and preventive, and primary dental services. The Expanded dental option described earlier in this section adds benefits for prosthetic and complex restorative services and orthodontic services.
3. Vision, Hearing and Expanded Dental Option
The Vision, Hearing and Expanded Dental option adds coverage for certain vision care services, and all the benefits of the expanded dental option described earlier in this section.
Vision Services
The Vision, Hearing and Expanded Dental option adds coverage for certain vision care services to identify and correct refractive error. Benefits are provided up to a maximum payment established for each covered service. These services may be obtained from an optometrist, optician, or ophthalmologist. The following services are covered:
- an eye examination once every 24 months (you pay $35 copayment);
- one set of frames every 24 months ($75 benefit maximum);
- one pair of single ($50 benefit maximum), bifocal ($75 benefit maximum), or trifocal ($100 benefit maximum) eyeglass lenses every 24 months; or contact lenses of any type ($100 benefit maximum), instead of eyeglass lenses, every 24 months.
Hearing Services
The Vision, Hearing and Expanded Dental option adds coverage for certain hearing care services to identify and treat hearing loss. Benefits are provided up to a maximum payment established for each coverage service. The examination may be obtained from an audiologist or doctor of medicine.
The following services are covered:
- an examination once every 48 months (you pay $35 copayment); and
- hearing aid(s) and other related hearing aid services such as selection and fitting every 48 months ($1,200 benefit maximum)

Flexible Reimbursement Accounts*
Section 125 of the Internal Revenue Services’ (IRS) Code allows employees to choose certain pre-tax benefits as an alternative to taxable monetary compensation. Employees are eligible to enroll within 31 days of employment. This is for a FY plan year 7/1/07 - 6/30/08 program.
Medical Reimbursement Account
This account allows an employee to set aside pre-tax dollars to pay for medical, dental, and vision care, or other eligible expenses, that are not covered by the health plan. Employees are eligible to enroll within 31 days of employment.
Maximum amount during a plan year:
$5,000 - plan year maximum.
$10.00 is the minimum contribution per pay period.
Dependent Care Reimbursement Account
This account allows you to set aside pre-tax dollars to pay for eligible dependent care expenses, such as child care. Employees are eligible to enroll within 31 days of employment

Retirement
Virginia Retirement System (VRS) - http://www.varetire.org
Full-time and part-time classified employees participate in the Virginia Retirement System (VRS) as a condition of employment effective on their employment date. Faculty may elect to participate in VRS or an optional retirement plan (see ORP). For faculty, participation in a retirement plan is also a condition of employment. You are vested after five years of VRS service.
VRS is a defined benefit plan with a formula to calculate retirement benefits. The VRS contribution is equal to five percent of the employee’s creditable compensation and an additional employer contribution. Old Dominion University makes the employee contribution as well as the employer contribution. VRS benefits are paid in addition to any other retirement income you receive, including Social Security.
Optional Retirement Plans (ORP) FACULTY ONLY
As an alternative to the Virginia Retirement System, the Commonwealth of Virginia and Old Dominion University offer full-time faculty members a choice of (2) optional retirement plans effective July 1, 2004. . ORPs are defined contribution plans. Old Dominion University contributes 10.4 percent of the contract salary. Faculty determine how the contribution is to be invested. Disability retirement is not provided. Faculty are vested with the first contribution. Faculty cannot contribute to any of the retirement plans. Optional retirement plans available are:

Full-time employees of Old Dominion University have life insurance provided by the Virginia Retirement System (VRS) effective the first day of employment as a condition of employment. It provides two kinds of insurance - life and accidental death and dismemberment. The amount of insurance coverage is equal to the employees’ annual salary rounded to the next highest thousand, and then doubled. An amount equal to four times the employees’ salary is payable in the event of accidental death. Coverage is also provided for accidental blinding or dismemberment. Premiums are paid by the University.

Additional optional life insurance is available and is administered by the Virginia Retirement System (VRS) and underwritten by Minnesota Life Insurance Company. It is available for employees and their spouses and dependents. Premiums are paid by the employee through payroll deduction.
Optional Long-Term Care
Insurance
Long-term Care Insurance is offered to full-time employees, their spouses, parents, and parents-in-law. Employees enrolling within 31 days of hire do not have to complete a health care questionnaire. Long-term care is the care you need when you require assistance with day-to-day functions like dressing, eating, and getting in and out of bed due to medical problems caused by illness or accident. Optional Long-Term Disability
Long-term disability income insurance helps to protect your income when, due to a covered illness or injury, you are disabled. It has been designed to cover a disability sustained on or off the job. The plan provides UP TO 60% of your basic monthly earnings after 180 days of disability.
Optional Personal Accident Insurance
Personal Accident Insurance is available for the employee, their spouse and dependent children and helps protect against losses due to accidents. A full benefit is paid for accidental loss of life, loss of use of both upper and lower limbs, loss of both hands, feet, eyes, loss of speech or loss of hearing in both ears. Partial benefits are paid in the event of loss of any one hand, a foot, eye, both legs or both arms.
Optional Pre-Paid Legal Plan
A Pre-Paid Legal Plan which for a monthly payroll deduction of $19.20 provides legal services such as traffic court representation, first offense DUI court representation, will preparation, credit/warranty disputes, financial contract or lease review, uncontested divorce, uncontested adoption, and purchase or selling a primary residence.
Tax Sheltered Annuities
Section 403(B) of the Internal Revenue Service (IRS) Code authorizes full-time and part-time employees (who minimally work 20 hours per week) of Old Dominion University to participate in a payroll deduction, pre-tax, tax deferred supplemental retirement plan known as a Tax Sheltered Annuity. TSA’s provide employees with a retirement savings plan to supplement other retirement programs such as the Virginia Retirement System (VRS), Optional Retirement Plan (ORP) and /or Social Security.
Deferred Compensation Auto-Enroll for State Employees
If you became a salaried state employee effective January 1, 2008, you will automatically be enrolled in the Commonwealth of Virginia 457 Deferred Compensation Plan.
The 457 Plan automatically defers $20 per pay period from your paycheck along with a $10 employer cash match. Your deferral begins approximately 90 days after your employment date.
Employer Cash Match Contributions
Faculty and faculty administrators on an annual contract and full and part-time classified employees who have a tax sheltered annuity (TSA) (403b) or the Commonwealth’s Deferred Compensation Plan (DCP) (457) are eligible for an Employer Cash Match contribution. An eligible employee’s contribution to the DCP or a TSA of $40.00 or more per pay period will generate a $20.00 cash match contribution. Employees contributing less than $40 per payday will receive a cash match of 50 percent of their contribution. (Minimum employee contribution is $10.00.).
Workers' Compensation
All employees (full and part-time) are protected against on the job, job-related accidents and injuries by Workers’ Compensation. Workers’ Compensation will pay medical bills and reimburse for lost wages in the event of a covered accident/injury.
Tuition Assistance
Tuition Assistance is available for Faculty, Faculty Administrators and Classified employees who have been full-time for at least one year and make less than $75,000.00 per year. Human Resources will pay for a maximum of 15 credit hours for the year for Classified employees - six credit hours in the fall, six credit hours in the spring and three credit hours in the summer. For Faculty and Faculty Administrators Human Resources will pay for 9 credit hours – 3 credits in the fall, spring and summer. Tuition Assistance is also available for dependents and spouses of full time employees.
